Taelyn Rae Holmes, 21 days old, of Cedar Rapids, passed away Friday, April 15, 2016. Memorial Services: 10:30 a.m., Wednesday, April 20, 2016, at Stewart Baxter Funeral & Memorial Services, Cedar Rapids. Survivors include her parents, Erin Holmes and Edward Jacobs; siblings, Krystal Bohannon, Jazmine Ross, Keeshawn Jacobs, Keeaira Jacobs, and Logan Cassell; grandparents, Christine Holmes (Cody Wade), Terry Bradish (Michelle Frake), and Annie (Edward Mitchell) Jacobs; great-grandparents, Agnes Holmes, Suzanne Bradish, and Zeola Mitchell; great-great-grandmother, Betty Warner; adoptive family of Amy Jo (Dan) Wolfs; and many aunts, uncles, and cousins. She was preceded in death by several great-grandparents; uncle Anthony; and cousin Wade McMahon.
